Nurse Case Manager
OverviewOverlook Medical Center, a member of Atlantic Health System, is seeking a full time Nurse Case Manager for the Case Management Dept. Hours: 8:30 am- 4:30 pm; Weekend as needed. On-Call required.Responsibilities
Essential Job Functions:
The selected candidate will coordinate and facilitate patients through the acute care hospitalization; manage all aspects of discharge planning and perform quality screenings.
Candidate will demonstrate and embrace the PRIDE core values (Professionalism, Respect, Involvement, Dignity and Excellence) in every aspect of their job.
Candidate must have excellent communication and computer skills.
Qualifications
Education:
BSN preferred
Required License:
Valid New Jersey Nursing License Required
Related
Experience:
At least two years of acute care experience required.
Case Management, Managed Care and /or Home Care experience preferred.
